Duties and Responsibilities:
The Case Manager is responsible for coordinating and providing comprehensive services to meet patient needs. Duties and responsibilities include:
- Evaluating patient service plans and recommending changes as needed
- Following up with patients to ensure appropriate health services are being provided and monitor enrollment
- Smoothly coordinating services between providers and payers
- Collaborating with other departments for continuity of care
- Helping to identify new resources and assist with the development of referrals
- Utilizing sound clinical judgment and best practices
- Documenting case management activities
Work Hours and Benefits:

Qualifications:
To be considered for this role, candidates must possess a minimum of a Bachelor’s Degree in Nursing, Social Work or Health Administration and be a Registered Nurse or Licensed Social Worker in the State of Oregon. Knowledge of case management models, Health Information Systems, and The Joint Commission is preferred. The ideal candidate must be organized, detail-oriented, and possess excellent communication and problem-solving skills.
Training and Education:
Training on our systems, processes, and policies may be required.

They may also provide education and counseling to individuals and their families, attend IEP meetings, coordinate discharge planners and ensure timely follow-up from other service providers.
